River Valley HomeCare offers Medical Social Services through licensed medical social workers (MSW). The MSW provides counseling, help with financial, social and emotional concerns as well as information about community resources.
Medical social services include but are not limited to:
- Patient assessment of social/emotional needs
- Patient assessment of financial needs
- Evaluation of home environment
- Community resource planning
- End-of-life support
